What is the process of enrolling with recurring credit card/debit card payments?
Once we receive your completed form, we will charge the credit or debit card you provide to us, the agreed amount on the date disclosed to you monthly. If the credit card or debit card charge is declined, we will make an attempt to contact you for a resolution. If unable to resolve, the membership benefits will be canceled within 3-5 business days.

How long does the enrollment process take?
After verification, it could take up to 7 days for finalization.

What if I need to change the credit card/ debit card on file for my recurring monthly membership payment?
If you need to change the credit card or debit card on file, simply stop into the Wyandot MedSpa and fill out an updated recurring credit card payment authorization form and present the alternate card for input.

Can I cancel if I change my mind later?
Yes, you have the right to cancel your membership at any time, but we do require 30 days’ notice.

Do I need to renew my membership annually?
If you paid for a full year in advance, your membership would need to be renewed at the end of that term. However, if you select the automatic recurring payment option using a credit or debit card, your membership will resume unless you contact us to cancel.
